JPJ is cheaper if you are in Malaysia and have time. RM 150, in person at a JPJ branch, valid for 1 year. For Malaysian residents who plan their travel in advance, that is the cost-optimal choice.

How a Malaysia licence is received abroad.
Malaysian licences are well-received in ASEAN and most of the world. At Hertz Bangkok, Sixt Melbourne and Avis London, agents recognise the bilingual MY card and process the rental smoothly. The IDP becomes important for two reasons: insurance (an accident without one voids most travel insurance policies) and edge-case bureaucracy (some rural-airport rental desks still ask).
ASEAN exception: Malaysian drivers do not need an IDP for Singapore, Thailand, Brunei, Indonesia or Vietnam under the ASEAN Driving Licence Agreement. In practice, Bali rental shops still check, Thai scooter rentals still ask, and a UN-format booklet shortcuts the conversation.
Outside ASEAN — Australia, New Zealand, Europe, North America — an IDP is expected at the rental desk and required for traffic-stop documentation. The S$66 (~RM 290) we charge is trivial compared to a missed Italy road trip.
