We have seen rentals delayed (not refused, but slowed by a manager check) at Sydney for travellers without one.
Outside the cities — Cairns, Hobart, Darwin, Alice Springs — local agencies are slightly more flexible, but the booklet shortcuts the conversation entirely. For multi-stop road trips through the Red Centre or along the Great Ocean Road, you do not want to lose ninety minutes at the desk negotiating with the rental supervisor.
Australian state police can request an IDP at a roadside stop if your licence is not in English. Penalties for driving without proper documentation start around AU$300 and reach AU$2,200 in NSW for repeat or compounding infractions. The IDP itself is small change next to that risk.
