Hertz, Avis, Sixt and Europcar at Madrid Barajas, Barcelona El Prat and Málaga airports check IDPs systematically for US, Canadian, Brazilian and most Asian licences. We have seen rentals refused at Barcelona El Prat for travellers who arrived with only their domestic licence.
Local agencies like Goldcar and Centauro are slightly less strict, but the risk of a refused rental on the first day of a road trip is not worth the $99 IDP cost. Bring it.
Spanish traffic police (Guardia Civil) routinely stop foreign-plated rentals on coastal motorways during summer. They will ask for both the IDP and the original licence. Carrying only one of the two is treated the same as carrying neither.
